New Medicare ID Cards - Verify Your Address

Medicare is mailing you a new ID card with a number that is unique to you. Social Security numbers are being removed from Medicare ID cards to prevent fraud and to keep your identity safe.

The estimated mailing date for Ohio is after June 2018.

What you need to do:

Medicare uses the name and address you have on file with Social Security. Contact the Social Security Administration (SSA) to make sure your address is up to date.
